· Pre-registration is strongly recommended but not mandatory by calling the Senior Center at 508-529-4558. Time slots will be filled starting at 9:15 a.m.
If possible, please click this link to download and print the flu insurance form . Complete the flu insurance form and bring it with you along with all your insurance cards on the day of the clinic. You will also find information statements on the flu shot and the mist by clicking the blue links.
The Upton Board of Health is mandated to supply the Vaccine Information Statement forms to each resident receiving the vaccine, please read the form carefully. Completing these steps will help speed up the time you will need to spend at the clinic.
There are no age restrictions; all are encouraged to receive the vaccination.
