The Board of Health will be sponsoring a flu clinic on Monday, October 27th, from 11:00 a.m. to 1:00 p.m. at the Upton Senior Center located at 2 Farm Street. Pre-registration is strongly recommended but not mandatory by calling the Senior Center at 508-529-4558.If possible, please print the flu insurance form which can be found here, fill out the form and bring it with you along with all your insurance cards on the day of the clinic.
The Board of Health requests you read the vaccine information forms about the shot and about the mist.
Completing these steps will help speed up the time you will need to spend at the clinic. There are no age restrictions; all are encouraged to receive the vaccination.Remember to wear short sleeves.
